We run a 6:45 a.m. huddle and I require a JHA before tools come out. I want a 3x5 pocket card that hits task, hazards, controls (include LOTO and fall protection checks), permits, and crew sign-off so I can spot-check and file at shift change — anyone have a clean printable template that gets used in the field?
Same pain with a 6:45 a.m. huddle — . I resized OSHA’s JHA sample into a 3x5: top line for task, three hazard bullets, “LOTO verified?” and “FP anchored?” checkboxes, permit #, and five crew initials; we print two-up on 5x8, cut, and spot-check at shift change. Base form is here: https://www.osha.gov/sites/default/files/publications/osha3071.pdf — want me to share the 3x5 layout?
I hear you on the JHA front, it’s like trying to fit a giraffe in a compact car! Consider using a format from an app like Safety Meetings that can generate those pocket cards on the fly… Anyone tried that yet?
